If you need a copy of your medical records from your prescriber telehealth visits, or you'd like them released to any third-party of your choice, here's how to request them.
- Email Wheel, the telehealth provider group that manages your Prescribing visits, at support@wheel.com to submit your request.
- Let them know you'd like your records sent to yourself or released to a third party and Wheel will send you a release form to complete, this authorizes them to release your records, either directly to you or to the third party you specify.
- Once you return the completed form, Wheel will process your request and send your records accordingly.
Please note: This is different from your Omada in-app Care Plan, which only shows a summary of your visits, medications, and progress, for an official copy of your medical records, always contact Wheel directly using the steps above.
