Inform your PT that you would like to receive a kit so they can send you a survey to input your shipping information.
Once your PT has confirmed they sent you the survey, navigate to the home page on your Omada for Joint and Muscle Health app and open the today tab at the bottom of the screen. You will be prompted to complete a survey that requests you to input the address you would like us to ship your kit to. If you are unable to locate the survey or have technical difficulties while completing the survey, please reach out to our support team at support@omadahealth.com.
Select “Start”
Select “Next” to enter the address you would like us to send your kit to.
After entering your address, select “Next” to complete your survey and your kit will be shipped to you shortly after.
