Omada Health will replace a device provided only by Omada Health, as needed, during the first year of participation in the program.
"As needed" is determined by the Omada Health Participant Support Team in the case of a scale, and a blood pressure monitor, or as determined by the BioTel Care Support Team or Greater Goods Support Team in the case of a blood glucose meter.
Devices malfunctioning past one year of participation will be replaced once at the discretion of the Omada Health Participant Support Team, BioTel Care Support Team, or Greater Goods Support Team.
After one replacement has been administered past year one of participation, a replacement cannot be administered, per this policy.
