Your Omada provider may ask you to share medical records to assist in your care plan like medical imaging, referrals, prescriptions, or clinical notes. To securely share this information, follow the steps below
- Look for an e-mail titled "Securely upload your medical records to Omada." Click "Complete".
- Upload a .jpg, .png, or .pdf file from your phone or computer. Select the type of record and click "Complete".
