If you sign up for an Omada program through Amazon's Health Condition Programs, Amazon will receive information about the status of your application (i.e., whether you have applied for specific programs and when, whether the application was accepted, and if not, general information as to why), your enrollment status in the program (i.e., whether you’ve applied, been deemed ineligible, enrolled, or disenrolled and whether you’re active in the program) and changes to that status. Amazon will also receive summary information about certain milestones in the program to display in your Amazon Health Care Programs Services account, such as confirmation of your acceptance and encouragement to start the program.
Amazon uses information from Omada to provide you with appropriate Amazon customer support specific to Omada programs, to update information in your Amazon Health Condition Programs Services account, and in certain circumstances, to remind you of next steps with Omada.
Amazon also receives certain aggregated, de-identified information from Omada, but this information does not include personally identifiable information from you.
