Congratulations on opting into Omada’s complimentary CGM offering! We hope you are excited as we are about this unique opportunity. 

Two (2) complimentary FreeStyle 14-Day CGMs are provided to Omada members who:

• Are eligible for, and opted into, Omada’s complimentary CGM offering
  • If you are unsure if you are eligible, please message Omada Support by clicking here.
• Have a valid FreeStyle Libre 14-Day CGM prescription on file with our pharmacy partner, Byram Healthcare. 
  • Your first complimentary CGM will ship from Byram shortly after their pharmacy department receives a FreeStyle Libre 14-Day CGM prescription.  
  • Your second complimentary CGM will be shipped 6 months later. 
• Have a compatible smartphone that can download the required CGM app, LibreLink.

Once you receive your first of two complimentary CGMs, follow these instructions or the Connect Your CGM Guide (included with your complimentary CGM) to sync your complimentary CGM data with your Omada app.
